You may request your disciplinary history from the Disciplinary Board of the New Mexico Supreme Court by completing the form below or by downloading the PDF form and sending it to us via postal mail or fax. 

There is a $10 administrative fee for this service if this form is sent to us via postal mail or fax. This fee is waived for completing this form online below.

IMPORTANT NOTICE:

Only the attorney can make this request. By submitting this form, you are waiving your right to confidentiality pursuant to Rule 17-304 NMRA.

This is NOT a request for a certificate of good standing. This must be obtained directly from the New Mexico Supreme Court on their website here.
